Health Forms
Health History :
Health History forms are required for girls/youth at all Girl Scout related activities including troop meetings, Service Unit events, trips and camps. These forms should be kept confidential by adults in the troop. Health History form must also be completed by adults when they are participating in a high-risk activity or overnight. Forms must be completed annually and updated when there are changes. A physical is needed along with the Health History form for any Girl Scout activity that is longer than 3 nights for both girls/youth and adults.
- Health History Form (required for girls/youth and required for adults for high risk activities and overnights)
- Girl Health History With Physical (only required for trips or camps 3+ nights)
- Adult Health History Form with Physical (only required for trips or camps 3+ nights)
Medication Permission:
Girl Medication Permission form is to be filled out by parent/guardian giving permission to the troop leaders or event staff to give any medications to girls including personal prescriptions and any over the counter medications. This form is required for any trips over 8 hours and overnight activities.
